I meant to send off my form to the BATFE last week to start the process on a SBS or SBR. I had a few emergencies pop up and forgot to get it done. I'm hoping to send one of them off tomorrow.
I found a couple of sample forms on a website that showed how people filled them out (can't remember what site I found them on). The questions seem obvious enough, but I still feel better looking at an approved sample. Anybody have a link I can view?
Do you normally print it from the internet or request that they send you original forms?

If you print these out make sure you copy the front onto the back so it is one sheet per form. Submit (2) forms. Hope this helps you out.:) Bill

If I'm using my Trust, which box do I check on question 2. Then do I use box 9 or 10?
Is a Trust considered an individual or business entity?

Do I need to do the Certificate of Compliance in addition to the Form 1?

Yes. Send in the following:
(2) copies of Form 1
(1) certification of compliance
Corp box on 2
Trust name on 3b
Your name on 9
Your name and title "trustee" on 10

There is a thread on AR15.com about possible Form 1 problems (see link below). Someone said they used the Form 1 on ATF's website and it was rejected because the approval block is on the second page, not the first. Might be worth looking into before you send one off.

That sucks. Now I don't know which place to get the form from.
If all else fails, call up the ATF and request a copy or three.

I think the problem is with the E-Form 1 on the ATF website. The paper Form 1 should be fine. I am going to order a few from the ATF distribution center. It is free and they usually take just a week to get them.

If you send in a form with errors, do you lose your $200 or get another shot to fix it?

You get a chance to fix it.
I had my first form 4 come back. I had it fixed and on FedEx overnight back to them the same day that it arrived. When it happened, I did not even think about losing my money, I was worried about being put at the back of the list. Since my transfer had been pending for about 3 months by that point, I was nervous. I called and talked to the person that put the note on what I needed to fix and he said that it was approved pending that change. I had my approved forms back in less than a week.

I'll use the Title II forms while I wait for my ATF ones by mail. Now that I know I won't lose my $200 if I make a mistake on the form, I'll probably do both my SBS and SBR at the same time.
If I submit two, should I send them in different envelopes?

I received my Form 1's from the ATF today. Is there a template for printing on these or what have others been doing to "print" them out?

Don't know of a template. I just use a typewriter I have at work. If you find a template, please post a link for future use.

I don't have a type writer. I don't think it would be too hard to make a template in Word. I feel like hammered shat right now. Once I get over being sick, I'll try to throw one together.

I used the forms from titleII or some that I found on AR15.com. Several of them are pdf files that you can fill in the blanks. Some of the boxes have drop downs. A lot easier then filling out forms by hand. Take about 3 minutes from start to finish.

I have little patience and gave up on the template for now. I just used the TitleII forms instead.
I think I boogered up my form.... In the box about additional markings on the weapon, I left it blank. I read on arfcom that most people put their "Name, Location" that would be engraved on it. Then somebody posted that a BATFE reviewer advised that that box should just be left blank. I left it blank, but now I'm worried that I messed up.
Any ideas?

You should be fine. I haven't used that area and both of mine are going through the process with no problems so far. Should know more in a week or so.

It's only been a few days, but it feels like an eternity. I thought y'all said the first stamp was the worst? The first one came back like a boom-a-rang. This one is gonna be the killer. I hate waiting.

Still waiting on both of mine to come back. The first check is cashed and in the process. Should show up sometime next week if not the week after. The last one will probably be the first of March. Waiting does suck.

I feel your pain. Fortunately the new WV NFA Branch is much faster. I remember waiting 3-6 months for forms to come back when the branch was in DC.

Dr how long ago did you send in your forms? Most I have seen have come back within a month or a little more (30-35 days from start to finish).

My NYR should have been to gain some patience. I seem to run short.
They cashed my last check a few days from the date I mailed it. I hope they didn't lose it.:lookaroun

If it hasn't been over 3 weeks I wouldn't worry about it. My last form 1 check took them around 2 1/2 weeks to cash.

I am still waiting on 2 form 1's to come back. But during that time it has allowed me time to purchase the other things I need to finish them besides the short barreled upper. That will have to wait until I have the form 1 in hand.
If they stick with their current time line I should receive one form this week and the other next week. But I am not counting on anything.
But waiting does suck.

Finally!
Just checked the bank website and the check has finally been cashed. Took 15 days from the date it was mailed. I would think they would be a little more excited about taking my money.
I should definately have the SBR ready for the Spring (bullet) Fling.

I never got around to sending in the SBS application. I'm now feeling a little more motivated to get it done. Hopefully it will go out within the week.

I did find out a little information while talking to a nice NFA inspector last week.
If you plan on building anything on a form 1 come up with a unique serial number. Don't use 001, 0001 1234 or something along those lines. It seems everyone uses those and NFA looks up your forms by serial number. So if you want them to be able to find your forms use something that only you would use. Of course I used 0001 because it was the first one I decided to do. I won't make that mistake next time.

Now the TitleII site has PDF forms that will save your personal info and download to your computer. However, that form puts the ATF signature block on the top of page 2. :smack:
I'm afraid to try it even though others post that it was accepted. I just followed the old link on TitleII and used that form. It worked last time so why not give it another go.
