Form 1 and Form 4, order of things...

So, I bought a suppressor recently, and am going through the form 4.

Steve (Cold Hand Arms) walked me through it and it seems easy enough.


I decided that since I have to go to the Sheriffs office to get fingerprinted, I might as well SBR one of my lowers so I can get that 10.5 I have always wanted.

Enter the Form 1.

Here's my plan, I just need to know what steps are out of order and have a few additional questions.

I have:
ATF Form 1
ATF Form 4
4 passport photos
Two checks for $200 each
4 FBI / ATF fingerprint cards (the ones with the ATF address on them)


I am planning to go to the Sheriffs office tomorrow and get fingerprinted. After these 4 cards are filled out, I take them home and mail them off the ATF (separately, form 1 and form 4) with the check(s).

After they receive them, they will contact the local Sheriff and have them sign them and send it a copy back to me.

At that point I can pick up my suppressor (form 4), and I can also get my lower engraved properly and order a 10.5.


Is this correct? or does the sheriffs office do more? Do I have to get it signed? etc... I need to know the flaws in this plan... hehe


I read somewhere that the papers have to be "copied" as double sided? Is this still true?

While you are at the local sheriffs office, he needs to sign your forms. Ask him to print his name near that signature. ALSO ask him to fill out the address of the office you're at with the phone number. I just had mine done and had to all of that myself....except the sheriffs signature of course.

The forms and check can be sent together. They do not have to be seperate. You can write the check for the $400.00 and send it all in one bundle.

The money and paperwork first go to Atlanta where they remove the check and then they forward everything to W.VA to the examiners.

It's not going to make any difference in the approval times, but the Form 1 and check CAN be sent directly to W.VA, but thats up to you. Don't worry, either way they'll get your money.

The CLEO has to sign the forms before the forms are sent. BATFE will NOT approve without that signature.

Yes, the forms do have to be double sided. Single sides will be rejected.

The form 4s I have that I got from Steve are single sided. His signature is on them... so I have to refill them out and get his signature again before I send them off? Can I do this after the Sheriffs office visit? or will that need to be signed before the Sheriff signs them?

Sorry, but I have to jump in here.

Form 1's do not go to GA first. they go to Martinsburg, WV. When i called them to check everything for my first form 1, it was explained to me that they will look over the form 1 before cashing anything so that they can make sure that everything is correct and "you're not trying to build anything that you can't own." then the check is sent to GA for cashing, then processed.
I asked the gentlman what happened if I sent it to GA, he explained that it would just take longer because they would have to send it to WV for pre-processing and then back to GA and then back to WV.

Check the tops of the forms.
