What's the most annoying thing about carrying a Makarov? For me it boiled down to the typical European heel-mounted magazine release which had two problems: 1) it was a ***** to change magazines because a) heel-mounted, b) heavy spring, and c) sumbitch was sharp! And 2) being sharp, it would wear out cover garments from the inside and mark up furniture. The solution I found was the FAB Defense PM-G Makarov PM/PPM Magazine Release Grip. It converts the magazine release to a sliding button AR the top of the grip (available in left or right), flush more ergonomic, and has an extended beavertail to prevent "slide bite." The grip is also available in black, olive drab green, and flat dark earth. Mine is the FDE and it rides very comfortably in a Versacarry Commander OWB holster.
A distraction was the unavailability of good defensive ammo. All I could find during the year long "15 days to flatten the curve" was steel-cased ball. That has now resolved itself with PPU brass-cased ball and Hornady Critical Defense nickelplated-cased FTX ammo showing up at the LGS, in this case Sooner Guns.

UPDATE: The Versacarry Commander holster tended to move the safety out of the "Safe" detent. I gave that holster to my daughter for her KelTec P-11 and got a left-handed version for when I carry the Makarov as a back-up. For right now, I'm back to using a Kydex IWB for the Mak.

Interesting solution, but what’s the benefit of carrying a Makarov over offerings in the much more common 9mm Parabellum?

I’ve got a ij70-17ah which doesn’t wear out clothes, but still has the euro style mag release. It’s also a double stack .380 so decent capacity and plenty of ammo options. It’s a pretty decent carry piece in my opinion, but I still tend to carry my g26...

Oh I dunno; but I have been known to carry a Walther PPK in either .380 or .32 ACP from time to time. They are comfortable, I shoot them well, and they are just so dang cool.
