Likely you pushed the slide release with your index finger as you racked it and likely had pressure on the release as you passed the slide past the breakdown notch, popping the slide release out. It happens. Check the slide stop by looking at it throout the ejection port with the slide locked back and make sure the stop is not broken off. That also occassionally happens.
