Shot many CQC rooms with my Beretta M9s in training over the years of active duty. Always had a lot of fun doing the training, feel as though it was generally well thought out and executed. First time I was forced to put that training to use in a real life situation, the gun was discharged into the target not more than 4 feet to my right. No time to get a sight picture and was fired from mid chest into the Adversary. Had many other situations over the years in confined spaces with the trusty M9, never, that I recall did a sight picture ever occur. Used a Crimson Trace for the last three deployments, great piece of kit and certainly worth it's weight in gold. My wife Jen, practices the mid chest hold as well. I have confidence that should the need arise, her crimson trace grips and a good solid carry will allow her to maintain control of the weapon and engage the target if necessary. She trains with a Beretta 92FS (thank you @Pokinfun) as a bedside gun and carries a G42 AIWB when out and about. Training is the key and not everyone does it. Sorry for the ramble, pain meds from the tooth extraction ya know.
