This is the 2nd time I've moved and sent off my notarized letter to OSBI and have got nothing back. Am I doing something wrong? Has anyone ever got a reply or new license?
You did the correct thing to stay in good stead with the law by doing the form from the OSBI website. Did you include the $15 for a new license with the updated info, if you wanted that?
Just says notify OSBI with notarized written statement within 30 days. (I've done this both time)

Yes, filled out the papers and sent the 15 bucks and had it in about 3 weeks.

Sent in my notarized paperwork and $15 about 3 weeks ago. Got it back in 5 days. Maybe I got lucky or hit them at a good time.
