Also, if they were using the gun as a prop and they were actually on the set, how did nobody think to unload it?
Even blanks can be deadly at short ranges. Just ask actor Jon Erik Hexum.

Oh, wait, you can't. He put a gun loaded with a blank against his temple and pulled the trigger about 30 years ago, with sad consequences for his family and friends. (Reportedly, the prop guys warned him numerous times to stop playing with the guns, but those warnings went unheeded.)

Guns only “go off” or “went off” when a booger hook is on the bang switch
Doesn't have to be a booger hook, just something that will apply enough force to the bang switch. If it's not drop-safe (like old Colt SAAs), falling out of a hand or pocket and landing right can get it to go off.

Brandon Lee was also killed by a prop gun.
On March 31, 1993, only days away from completing the film, Lee died during surgery when injured on the set of The Crow after being shot by a faulty prop gun that fired the tip of a dummy round that was accidentally lodged in the chamber. The film was completed by re-writing the script, CGI, and stunt doubles, and released one year after Lee's death to critical and commercial success. It is now considered a cult classic.

Well, there is another side of the story now, it was not a prop gun and was not during filming a music video. Sometimes I really wonder how many "accidents" are really accidents?

“He was at his apartment getting ready to go work and do a video shoot,” said Atherton. “We don’t know how, but the gun went off; the bullet entered his head. It was ruled an accidental shooting by the Montgomery County Sherriff’s Department in Texas.”
