You need your serial number, who you bought it from, the name you bought it in.

Error letter fixed and returned. The word "the" was left off the trust title. Don't you love how the government serves the taxpayer.

Were do you go to fill out one of these forms ? Do you have to have a lawyer do the trust or can i right it up? Are they a online place to go and do i need to buy the supressor first? Thanks

Personal choice. I used quicken will/trust maker while a friend of mine hired a lawyer to make his trust. They both look about the same.
If you want to build your own SBR/supressor then you fill out a form 1, if purchasing then a dealer will fill out form 4 for you
