You can submit it online, upload a passport photo and use a debit or credit card online. Did mine that way two weeks ago.
Would be nice, but I don't have a digital camera or smart phone and if I did, would not know how to "upload" anything.
Prior to my original post, I had explored the options and it appeared there were 3: Print an application, apply online, obtain an application and complete it. Given my circumstances I decided the best option, for me, was the last. I did not know where I could get one, so I ask. sedona answered the question.

you can go by library and go to osbi web site to print a sda application.
Go to the OSBI web site and od it on line. Take a digital picture of yourself and up load it per their instructions.
Thats what I did.
