Problem with Dillon Square Deal B

I have attached a picture below... you can see on the left of the bottom row the primers are pretty mangled... but the rest of them just have a moon-shaped dent in them. These are Federal Premium Match primers btw.</p><p></p><p>Now, right before this started, a thin black washer appeared to fall out of the press from the die area up underneath. The washer is very thin and is bent - sort of rounded.. it is also in the picture below. I have NO idea where this goes, but the press seems to be working fine without it and the primer problem was temporary. The hitch in the action is NOT temporary...
