If I can't do takeout or consume it in the restaurant, I'm not paying for it.
Prompt service, not too much hovering, no touching me on the arm or shoulder (or anyplace else), no obvious flirting--all of these enhance the tip size from me. I am pretty much of an over tipper based on the occasionally listed "appropriate amounts" on menus, etc.
Tipping always seems to bring out a lot of emotion on internet forums. Many times those people who appear to have the least control over their own lives and emotions get really vocal and judgmental.
The way I figure it is that wait staff makes most of their money from tips, and if they do a good job I recognize it with a generous tip. I don't think I have ever tipped anyone based on their looks unless I was making plans to see if I could move the relationship beyond waitress/patron. Found that to be a dumb move pretty quickly.
