I don't see anything written on the on-line printable application that says you can't fold it. You just dont want your pictures bent on the corners.
I always sent my renewals in a 9" x 11" yellow envelope by certified mail. It costs a few extra $'s, but I figured it was less likely to get lost that way because of the larger envelope and the pictures less likely to get bent. And with certified mail I know exactly when it arrived at the OSBI and who signed for it. With my last couple of renewals, it only took approx. 60 days to get my license.

Just got my renewal in today (it was expired by 1 week)....14 days from send off to receiving it back from OSBI. I did mail in large envelope.
