Can't think of a specific movie, but I've seen people continue to "shoot" with the slide locked back.
Or someone cycles the action on a pump shotgun in a threatening manner, doesn't fire the round then moments later cycles it again and there is no round ejected.
Then there are the Jason Bourne or Liam Neeson type badass operators who grab a semi auto that's pointed at them at point blank distance and somehow field strip it with one hand in a split second and leave the bad guy holding a frame. Usually followed by a throat punch. Lol

This! Why does NO ONE carry with one in the chamber? Good guys, bad guys, doesn't matter - they all have to rack the slide before taking care of business. I must be getting old and cranky cause this one has started bugging me EVERY TIME
