I would have left. At this point, I don't pay until after my appointment because of things like this. How can you be behind an hour and a half on an 8:30AM appointment?
The Dr. could have been on call and had to report to the hospital for surgery.
We had a doctor who was always, and I mean ALWAYS, a couple of hours behind (she triple booked patients). Once GC said something to the receptionist about it and she made some kind of lame excuse like "Oh, it's just been really busy lately." GC said to her "Well, I can help you guys out with that." 
Said she gave him the funniest look. I gotta say I was surprised ... that's something that usually comes outta my mouth, not his ... 
Came home, told me to find him another PCP and never went back. That particular doctor's office isn't far from the house. In fact we drive by there quite frequently. Seems we weren't the only ones who got sick of the long waits as her parking lot is practically empty all the time now ...
