Catastrophic failure- what do you think happened?

It looks like the breech never unlocked, so instead of distributing the recoil impulse through the reciprocating mass of the slide and across the frame rails, it all got concentrated on the locking mechanism and down into a small part of the frame. The chamber looks like it's still in battery, and it should be at least partially unlocked when the slide is that far back.

He’s a lefty so both thumbs were on the right side. He’s still a little sore in the thumb area
