You can make it yourself.

Take a picture against a white/light gray background and print with a quality printer on some photo paper.

Just make sure the picture is the right size when you print it

Then cut it out and tape it to your application

I took mine with cell phone, sent to my email and then printed out on photo paper with the printer on my desk. I did have to print twice to downsize as the first attempt was a little too large.

You need TWO current passport grade pic's. Walmart, Walgreen, FedEx-Kinkos are some of the common places to get it done.
