Backing what is essentially a double trailer would take considerable skill!! If he doesn't start the outboard, would that be within the 'rules'?

I don't know. Look at the leg placement. I've never seen a horse with one leg coming out of their belly. But, the configuration doesn't look like a team either. Maybe one horse is being ridden?
It's a double hitch (2 horses) pulling the buggy. See the tongue sticking out between the two? If it were a single we'd see a "tongue" on this side of nearest horse.

Yeppers...

...I missed the center tongue, and I also missed the angle from which the photo was taken. Which is why the leg seems out of place. Zoomed in on the image and could even see the chest of the second horse.
