Type up a written word doc. Thank them for the opportunity, sign it and give it to your direct supervisor. There is absolutely no need to go into any details beyond that. Pull your supervisor aside and tell him casually what's up and that you're leaving. Also, give him a typed up document that's a little bit formal thanking them for the job, gotta look out for me and mine, another opportunity elsewhere, yada yada yada.

No need to burn bridges, you might have to retreat across them someday.

This is exactly what I did/do. My mil was an hr executive and told me to do this. You can also include an effective date and I’m not sure 2 weeks is the norm anymore. It really could depend on if your current employer wants you to hang around. They may walk you out when you give them notice.

No need to express regret, but you can express your enjoyment with working there up to now if you want, but you do NOT in any way need to go into details as to why. That is none of their business, and if you're not looking for them to make a counteroffer of some kind, it's pointless, IMNSHO.

I'd also be happy to send you a copy of mine, I turned in my notice 2 months ago - 1 month to go until freedom! lol

Yeah... I have to give 90 days notice per my contract.

Agree with much of what's been said above.
Think about it first and type up written notice to your supervisor.
Be polite, don't need to give them a reason.
Offer to work a 2 week notice, but be prepared in every way (financially and otherwise) for them to immediately escort you to the door, no matter how good of an employee you have been.

*Edited to add -- Might want to come in early that day and clean out your desk or work area and take any personal property out to your vehicle first.

It must be in the air, I turned in my 3 weeks today. It actually got a little heated. He's retiring at the end of may, it's the only reason I was looking for another place. this one was my top choice, better pay and benefits, but they're not going to hold it till the end then. I talked them into waiting till the 16th, which is when we'll stop seeing patients. I feel bad about leaving him in the lurch with the final 2 weeks, but it is what it is.
He left saying "we'll talk more about this tomorrow."
i immediately called the new place and told them I might be available sooner.
