Yup, BOK and FAA is who I use. My wife just had her debit card replaced (BOK). To replace the card in the future, there will be a $2 charge.

..and I think $2 is really no big deal to replace a card. Sure, they don't cost that much to make but there are expenses involved in processing the request for a new card, mailing the card, etc.
