I've never been a fan of using my sights to rack the slide...I'm partial to pinching the slide behind the knee and pushing forward. But if you can't squat, you could try pinching it between your knees/lower thighs, pointing down and away from you of course, and pushing forward.

this works for me.

Like others have said, hook the rear sights on something and push forward. With that said, your thumb is fubar but not your fingers. In a real deal, I would just place 4 fingers over the top of the slide and rack it. It will be uncomfortable, but it's the most reliable
