I have no idea what its used for in reference to Physicians, just know they use them

You sure it's the same one as the UPIN used for guns and not the NPI number every medical practitioner has?
UPIN (Unique Physician Identification Number) was replaced by NPI (National Provider Identifier) for physicians in 2007.
However, UPIN also stands for (Unique Personal Identification Number). This is the one you'd want to apply for.
I still get the occasional delay cause there is a felon in the northern part of the state with the same name. We have different middle names, but it's enough to get looked at twice.
