If you have a class 3 item in your own name, would you have to pay another $200 to have it moved to a trust? Would that be a "transfer" even though it still remains with the same person?

Yes, it would be a transfer and require a $200 tax. It is transfering from an individual to another entity. The fact that it is from you to your trust does not matter.
