Question on Renewal

In the renewal package it does not say anything about the "notorious extra" form that has to be filled out by TCSO or any other agency!
Will you guys enlighten me please.

That would be because there is no need to go to TCSO for renewal. Fill out the form, get new pics, and send the money. Go for the 10-year renewal and forget about it for a while.
