Why did the loading levers keep falling on the wheelguns? Are they supposed to do that? Seems like a major design flaw if that is the case?

The Walker was Sam Colt's first design (1847) after a decade long hiatus from the firearms world. He hadn't yet added the latch under the barrel for the loading lever. IIRC this was remedied in the Dragoon models a year or two later.

Another interesting thing about Walkers is that though they were referred to as "holster" pistols, that meant that they were intended to be carried in horse-mounted holsters rather than as a sidearm. The things are beasts.
