My Ft. Knox has both digital and dial. It's a redundant system. Can use either independently. Digital for ease of use and dial for after RedDog's EMP hits.

Mine is set on plastic shims. Wood seems like a horrible idea.

A thin rubber mat. Don't want the metal in contact with the concrete. A thick washer, if you want air space, but 1/4" is a bit excessive. IMHO
The red head anchors are worth their weight in gold. Only thing better is knowing where to put bolts when pouring the slab and having them permanently set. Of course, if you change safes, the bolts may not be set right.
